• Sub-topic 1: Strengthening the primary and community health and care system
• Sub-topic 2: Systemic approaches to modernizing the primary and community care sector
• Research Performing Organisations, healthcare providers, patient organizations and other relevant stakeholders
• Project consortium must involve at least three partners from three different THCS member countries (see call document for list)
• Patient and citizen involvement
• End-user involvement
• Compliance with Responsible Research and Innovation (RRI) and ethical standards
